EKG READING

Electrocardiogram or EKG – measures the electrical activity of the heart Electrical conduction

AUTOMATICITY – ability of the heart muscle to initiate an electrical impulse
  1. SA Node (normal pacemaker of the heart)
  2. AV junction
  3. Bundle of His
  4. Purkinje fibers

P wave – Atrial depolarization
QRS Complex – Ventricular depolarization
T wave – Ventricular repolarization
U wave – may or may not be seen; repolarization of Purkinje fibers; Hypokalemia
PR Interval – from the onset of P wave to the onset of QRS complex
QRS Complex – amount of time the ventricles depolarize

NURSING INTERVENTIONS:

1. Avoid bony areas/ scar tissues/ skin folds/ increased muscle mass
2. Perform a 10 cm shaving around the area; AVOID if possible
3. Dry the skin with the use of gauze
4. Remove excess skin oils EXCEPT for geriatric pts
5. Dry with gauze
6. Place the electrodes. Make sure the center is moist; otherwise, discard.
7. EQUIPMENT Trouble shooting

  • Horizontal axis - represents the time in seconds (sec)
    1 small square = 0.02 seconds
    5 small squares = 1.0 seconds
  • Vertical axis - represents the amplitude in millivolts (mv)
    1 small square = 0.1 mv
    5 small squares = 0.5 mv
    2 big squares = 1.o mv

NORMAL SINUS RHYTHM (60-100 bpm)

Sinus Bradycardia
atrial rate of 50 or <60>

Sinus Tachycardia
atrial rate of >100 bpm
due to anxiety, blood loss, Fever, Pain

SINUS ARRYTHMIA:

Paroxysmal Atrial Tachycardia (PAT) or Premature Supraventricular Tachycardia (PSVT)
>150 – 250 bpm
due to CADse, Mitral valve dse, Thyrotoxicosis
INTERVENTION:

  • Valsalva maneuver + Exhale slowly
  • Rest
  • Medications to sedate
  • Medications to slow electrical conduction to the heart (VERAPAMIL HCL)

Atrial Flutter
due to inflammation, fluid overload
INTERVENTION:

  • Medications to slow ventricular rate (DIGITALIS)
  • Anti arrhythmics – Procainamide HCL
  • Synchronized cardioversion

Atrial fibrillation
due to alcoholism
350 – 600 bpm
decreased CO
INTERVENTION:

  • Administer medications (Digitalis, Verapamil HCL)
  • Synchronized cardioversion

1st degree AV Block
damaged hearts due to ischemia, Rheumatic fever, Infection
INTERVENTION:

  • Atropine Sulfate (IV- 0.5 mg)

2nd degree AV block
Mobitz Type 1 AV Block/ Wenckebach
due acute RF, MI, Digitalis toxicity

2nd degree AV Block
Mobitz Type 2 AV Block
due to Pulmonary edema, Digitalis toxicity, worsened MI dse
INTERVENTION:

  • Atropine Sulfate ( IV – 0.5 mg)
  • Isoproterenol

3rd degree AV Block
Pacemakers (Permanent, Delayed, Demand, Fixed rate, AV Pacemaker)

Premature Ventricular Contraction (PVCs)
common in Type A personality
due to CADse, can precede dysrrhythmias, Acute MI/ Ischemia
INTERVENTION:

  • Lidocaine HCL (IV bolus, initially then thru Infusion)
  • Atropine Sulfate IV

COUPLETS – IV Lidocaine

Multiform/ Multifocal PVCs – Bolus Lidocaine

Ventricular Tachycardia
Due to mild Congestive heart failure
Tachyarrhytmia
>150 bpm / HPN
INTERVENTION:

  • IV Lidocaine
  • Procainamide
  • Precordial thump (obsolete)

Just what I hinted weeks ago, Freedom Watch, the institutional blog of the Center for Media Freedom of the Responsibility (CMFR), now regularly carries media monitors. Check out Freedom Watch for some of these new monitors.

And it also has something to say on the Gretchen Malalad-Bong Alvarez incident that happened yesterday. Video (including quotes from CMFR deputy director Luis V. Teodoro) below taken from Freedom Watch site.

I saw The Simpsons Movie recently, thanks partly to its hype, and because there wasn’t any better choice at the local cinema. I often catch the series on tv, and usually enjoy Homer’s idiocy. But sitting through an hour and a half of it was just too much, and I was more annoyed than pleased. Because I was expecting it to be a comedy feature, I find it uncanny that my laughs were few and far between. I knew it was supposed to be satire without trying to be, but the funnies and the plot were predictable, the ending cliche.

Through Homer’s brashness, the story unfolds to near catastrophe, threatening to break up the family and endangering the whole town of Springfield. But he soon recovers after an epiphany and saves the town the Bruce Willis way.

I’d rather go back to watching the shorties, or gawk at Viktor & Rolf’s creations in this:

Desperate.

The best things in life are free. Yeah! I believe so… being Free is actually what everyone wants right? Free… I am desperate to be free nowadays… Not that Im being imprisoned somewhere but the fact that I have been bothered a lot of things lately. One thing is my Career, I never really wanted to get into a Contact Center Industry but during the time I decided to Sign up the Contract more than a year ago. I could still remember those days that I am desperate to get out.

Now, I am in that situation too! I am Desperate to get out. Get Out. A lot of things are to be considered before I finally make my decision, I Dont know if this would be a right decision, all i know that it would be a healthy decision. I cant blame other people to feel the same. Can you just imagine working for the one of the world’s biggest outsourced centers and that a lot of people come and go.

Though we get to have the Biggest Compensation but the pressure is unbearable. I am not growing and I am not moving. I need to venture out. One friend recently told me that we are not being Valued. Valued? that’s not applicable in this d%mn center. Rumor has it that we will have an increase and the question is : “When is it gonna be?” It’s just like our commission and that was supposed to be released last 5th but where is it? GONE…36-48 hours?

That’s gone too! I am not moving.. what a waste of my time. Tsk tsk tsk… I am so frustrated thinking about those people who come and face their decisions to really get out. Should I be on the Trend as well? I just hope I can still muster the courage to go through it all. God help me.

July 2007 Physical & Occupational Therapy Licensure Examination Results

Congratulations to all board exam passers! :)

The Professional Regulation Commission (PRC) announces that 472 out of 1,119 passed the Physical and Occupational THERAPY Licensure Examinations given by the Board of Physical and Occupational Therapy in the cities of Manila , Baguio and Cebu in July 2007.

The members of the Board of Physical and Occupational Therapy who gave the licensure examinations are Dr. Reynaldo R. Rey-Matias, Chairman; Raul G. Agustin, Marylynn B. Querouz and Bernadette M. Reyes, Members.

It’s not quite a large number but at least, there are still people taking up these courses even with the outstanding demand for nurses.

I'm not be able to blog for about the next 5 days as I will be on a well-deserved vacation in Kuala Lumpur a.k.a. KL and the pre-colonial trading hub of Melaka a.k.a Malacca in Malaysia. This will be my 3rd time to go to Kuala Lumpur, 1st time in Melaka and 7th time to be in Malaysia but I still look forward to go to this beautiful country every time. This year is Visit Malaysia 2007 as they celebrate their Golden Year as an independent country this coming August 31.

The month of August will be filled with a myriad of parades, cultural performances, street shows and carnivals to commemorate Merdeka Day (Independence Day). It was on August 31, 1957 when history was made as the Union Jack was lowered and the Malaysian flag hoisted.

Malaysia has fervently embraced modern technology as it boasts the tallest twin towers in the world (the Petronas Towers). But the country still has some of the world's most remote jungle in Sarawak and Sabah and an atmospheric colonial hill station in the Cameron Highlands. Even in the capital KL which bristles with ambitious new building projects, one can still find exotic markets and plenty of local color.

And of course my favorite, The Malaysia Food Festival 2007 which coincides with my visit where visitors can expect unique gastronomical multi-sensory culinary adventures that will be experienced not only through delicious food but through competitions, interactive cooking demonstrations and celebrity chef appearances. There will be an overflow of mouthwatering, local and traditional delights like my favorite chicken satay, beef rendang and ais kacang.

For several decades, Malaysia has industrialized rapidly, transforming itself from an economy whose livelihood relied primarily on the production of mineral and agricultural export commodities into an economy dominated by manufacturing and services.

I encourage everybody to come and be mesmerized by a unique multicultural populace celebrating 50 years of nationhood in harmony and peaceful co-existence.

How do you make screenshots for your blog articles?

Whenever I write some guides or “how to’s” especially on Blog Tech Tips and on Wordpress stuffs, I always include screen shots because I believe that a picture is worth a thousand words. I can understand something better if I see some visuals and I guess it would also apply to other people.

Anyways, yesterday, my friend Kirby from Kirbitz asked me:

What tool did you use when you had the screenshot of the pages for your last post? Please advise on this one. Ive been looking for good methods for this one but im still using the CTRL+Prinst screen and then edit at Paint.

When I read this question, I smiled. Little does he know, I’m also using the Print-screen + paint combo! Hahaha. So here goes my guide on how I use this combo. ^_^

For this guide, here are the requirements:

  • MS Paint (If you are using windows as your operating system, you should have this)
  • Keyboard with a functioning Print Screen key (it’s the PrtSc key)

Step 1 : Find something you want to take a screen shot of

For this guide, I’m going to be using my blog homepage. After you found something you want, press the PrtSC or the print screen key on your keyboard.

Step 2 : Open Paint and Click on Edit->Paste

After pasting you will see this. Now you decide if you want to use this image or just a part of this image.
If you want to use the whole image, Proceed to Step 5.
If you want only a part of the image, Proceed to Step 3.

Step 3: Select the area you want only

For this example, I want to use only the Search box in the left side of my blog.

First let me show you the left panel of paint. The only buttons you need there are the two buttons at the top (star with dashed line and box with dashed lines). After pasting your “print screen”, the “box with dashed line” is activated. You can deactivate it by clicking on that box. You have to deactivate it in order to proceed.

After deactivating it, let’s activate it again.

I’m not playing jokes on you. The star and the box with dash lines act like a highlighter. Earlier, what was highlighted by the box was the whole picture but we only need the search box (or the part of the picture you want) right? So we have to deactivate it in order to loss the highlight on the whole picture and re activate it in order to highlight on the search box.

After you activate it, it’s time to select the part in the picture you want. You can do this by clicking on the top-left part on the “part” and dragging it down to make a box with dashed line around it. The part you want should be enclosed in a box of dashed lines.

After making the box with dashed lines, Copy it or Press CTRL+C.

Step 4 : Paste it on another paint program

You can run two instances of paint in your computer. Just open it again just like how you opened Paint before.

Paste it on the other paint program. CTRL-V.

Step 5: Save and you are done

Congratulations! ^_^ Now all you have to do is to upload it somewhere. I usually upload my pics at imageshack and tinypic since they both allow hot linking! hehe

Note: I use an online tool called Shrink Pictures to resize my pictures since if I don’t resize it, it will wreck my layout! haha

I hope this guide has helped you even if it’s just a simple guide.

Now this gives me an idea, if you want to know something and want me to write a guide for it, just tell me. If I’m able to do it successfully, I’ll make a guide on how I did it. If not, then, I’ll hide from you. haha

I had a root canal yesterday. It wasn’t painful at all. Only my jaws hurt because I had to keep my mouth open for long after every gargle. Tita Eileen couldn’t use a bite block because it might remove some of my brackets. I was fine the whole day yesterday, but not today since I woke up. It’s been hurting, but the pain is tolerable. It’s just hard to think. Also with this hot weather. *sighs*
